Request an appointment
If you need to see your GP, then you can request an appointment online using our online system. Our appointments are booked at 15-minute intervals.
Notice
It may not be necessary for you to attend for an appointment. You can submit an online consultation via the button below. You can submit details about your condition or your administration request and you will receive a response from the practice usually within 24 hours but you will be advised of the timeframe once you have submitted.
You may still call the surgery if you need to see a Nurse for appointments such as dressings, vaccinations and cervical smear screening ect., we will endeavour to give you an appointment time to be seen.
Submit an online request through Anima
Request an appointment online to see your GP by using our online system Anima. Click the link below to access Anima. You’ll be able to choose the type of appointment you need.
If you are unable to book an appointment please call the surgery on 01752 403206 and we will complete an Anima request on your behalf.
When you arrive for an appointment make sure you check in at the Appointment Touch Screen by the main entrance or reception if you prefer before you sit down in the Waiting Room. Do let us know in good time if you are unable to keep an appointment. If you suspect an infectious condition, tell the Receptionist who will get a side room ready.
If you wish to speak to the Receptionist in confidence there is a quiet area away from the main Waiting Room – please ask.
Types of appointments
At Church View Surgery, we offer different appointments to help you get the care you need as soon as possible.
Routine appointments
We aim to give you a routine appointment to see a GP within 2 weeks of receiving your online request.
Nurse appointments
We operate a booked appointment system for all clinics. When booking your appointment please let reception know what your appointment is for so that they can advise you when would be the best time for you to come and the length of appointment you require.
Urgent appointments
If your appointment is urgent, we’ll arrange for a GP to see you on the same day.
You can also call NHS 111 for general medical advice.
